Aeon Clark M.S.
FOUNDER/EXECUTIVE DIRECTOR

Aeon has over 25 years of experience in the human services field predominantly focusing on new program development for individuals with special needs. In 2012, he incorporated Unique Care Community Services. He provides visionary leadership to the organization while fulfilling his vision and mission. His goal is to make tailored support services accessible to those in need. Aeon holds a Master of Science degree in Organizational Leadership from Grand Canyon University, a Bachelor of Science degree in Psychology from Brooklyn College, and an Associate degree in Applied Science with a discipline in Human Services from New York City Technical College.

Tanese Talbert
DIRECTOR HABILITATION

With over 20 years of experience working with the Department of Education as a classroom teacher, the Department of Health as an early intervention program director and helping children and families with developmental disabilities Mrs. Tanese Talbert M.Ed. holds a Bachelor degree in health science from Lincoln University and a Master’s degree in Early childhood education. Mrs. Talbert is driven and passionate about her roles and  strives to lead her team to deliver quality services to families. 

Venese Talbert
DIRECTOR OF HABILITION SERVICES

Venese Talbert is a graduate with a Master’s in Early Childhood Education from Grand Canyon University. She also hold's a Bachelor’s in Human Service which she completed at Lincoln University and a Masters of Education in Educational Leadership from Grand Canyon University. Through the years of attending school and various positions working as a early education teacher and director of various educational programs she has had the opportunity to enhance her skills and competences that are necessary for a successful career opportunity in this field. These opportunities include hands on experience with children and families, preparation and implementation of program activities, as well as working with a variety of community organizations with respect to enhancing linkages and collaborations as well as coordination of serves for a diverse client population.

Kereece Binns
HABILITATION COORDINATOR

Kereece Binns is a well-rounded individual in the human services field. She has held positions such as a direct care professional, quality intellectual disability professional (QIDP) and a Medicaid Service Coordinator (MSC). She has a sense of passion and drive to serve a population of individuals with autism and developmental disabilities. She has made a positive impact in people's lives by helping each individual grow, gain skills, and work towards independence. Kereece enjoys her job because it allows her to be a part of something bigger than herself.  Kereece earned a bachelor’s Degree in Business Management and Finance and minor in small business management from Brooklyn College.

Renae Gross
RESPITE COORDINATOR

Renae Gross recently transitioned from a DSP to the Respite Coordinator in July 2018. Renae is a hardworking, self motivated, respectful and honest young woman who gets along with pretty much anyone because of her personality. She takes pride in what she does and complete any task given so each individual can excel and become a better person.

Stephanie Johnson
INTAKE COORDINATOR

Stephanie Johnson is an Intake Coordinator/Receptionist for Unique Care Community Services. Stephanie has an extensive background servicing people with disabilities. Starting as a Direct Care Trainee for O.M.R.D.D, continuing with different agencies and positions. Subsequently, Stephanie became a Day Hab Assistant then Specialist for F.E.G.S for 9 years. Stephanie gained certificates for conversational Spanish, sign language and participated in newer programs such as Saturday Respite and Comm Hab.
